Indiana Health Care Volunteer Registry

In 2015, Indiana’s Health Care Volunteer Registry was established in order to grant civil immunity to dentists and other health care professionals, under certain circumstances, who volunteer their services outside of traditional health facilities.

In order to be included on the registry, dentists will need to download and complete the Application For Inclusion On Healthcare Volunteer Registry By Individual available at the Indiana Professional Licensing Agency (PLA) website. The form can be returned to the PLA via postal mail (address available on form) or emailed to pla8@pla.in.gov.

There is no charge to be added to the registry. All information is stored online only, you will not receive a card or certificate verifying your status. You will have the option, however, to purchase a card, which you can use to promote your volunteer services.
